What it is
Thymosin alpha-1 is a naturally occurring 28-amino-acid peptide first isolated from thymus tissue. It modulates immune responses by promoting T-cell differentiation and maturation, enhancing dendritic-cell function, and shifting cytokine balance. A synthetic version, thymalfasin (Zadaxin), is approved in parts of Europe, Asia, and Latin America.
What the evidence shows
Human trials, primarily in viral hepatitis, sepsis, and cancer immunotherapy, have reported improvements in immune markers, viral clearance, and clinical outcomes in some settings. Meta-analyses of hepatitis B and sepsis are mixed but trend toward benefit. It has also been investigated as an adjuvant to influenza and COVID-19 vaccines, though results are preliminary. The overall human evidence base is real but smaller and less consistent than for approved drugs.
Risks & unknowns
Thymosin alpha-1 has generally been well tolerated in trials, with injection-site reactions and mild systemic symptoms being the most common adverse events. Because it is immune-active, there are theoretical concerns about autoimmunity or excessive immune stimulation, but these have not been prominent in the reviewed literature. Unregulated research products carry quality risks.
Legal / regulatory status
Thymosin alpha-1 is not FDA-approved in the United States. It is approved in several other countries for hepatitis B/C and as a vaccine adjuvant. In sport, thymosin alpha-1 is prohibited by WADA under S2 (peptide hormones, growth factors, related substances) at all times.
