What it is

FOXO4-DRI is a modified peptide that disrupts the interaction between FOXO4 and p53, causing p53 to relocate within the cell and trigger apoptosis selectively in senescent cells. The D-retro-inverso modification increases resistance to proteolysis. It is a senolytic candidate.

What the evidence shows

A landmark 2017 study in aged mice showed that FOXO4-DRI improved fitness, renal function, and hair density and reduced senescent-cell burden. The effect was senolytic and p53-dependent. No human trials had been published as of 2026.

Risks & unknowns

Senolytic therapy is a young field. Selectively killing senescent cells could impair wound healing, tissue repair, or immune responses if too many cells are cleared. Long-term safety in humans is unknown.
